High School Youth Ministry Registration

- Faith Formation

We will be using Lifeteen this year for High school ministry. Due to the situation with the virus, we will not hold live sessions at the parish this year until notified by the archdiocese that we can.

We will send you a message via Flocknote once we have more information for you.

We will provide materials online to help you form your child in the faith. And you can always call the office if you have any questions for us.

This form is designed for on campus sessions. We ask that you fill it out completely in the event that we can provide ministry on campus later this year. If there is information that cannot be completed put N/A in the text box.

There is a non-parishioner fee of $100.
